If you are unable to keep your scheduled appointment, please let us know so we can reschedule your visit to a more convenient time. On your first visit, please bring your insurance card, and arrive 15 minutes early to complete the necessary paperwork.
We will begin your treatment on your first visit so please wear loose clothing for ease of movement.
Before billing you, we will file a claim with your insurance company. If your insurance requires a co-pay, please pay it at the time of your visit. A referral from your medical provider is required before you can be seen by Rehabilitation Services. To schedule an appointment, please call (903) 877-7293.
We are open Monday through Friday: Adult Outpatient Clinic 8:00 a.m. to 4:30 p.m.
Pediatric Outpatient Clinic 8:00 a.m. to 6:00p.m.
We provide coverage for inpatients on Saturdays only.
