Employee Exercise Gym Information

We encourage our fellow employees, volunteers and students to exercise for good health! The Exercise Gym in 409B is open 24 hours a day for use. There are some time restrictions of use during week days. Neurorestorative uses this gym with their clients between 9-12 am and 1-4pm M-F. You may use the gym at any time if they do not have clients in the gym, however if they come to the gym to work, employees need to leave and come back at another time.

To get access to use the Gym:

• Print and sign the Release and Indemnification Agreement use of Exercise Facilities form.
• Print and sign the Employee exercise policy and agreement form.
• Bring the forms to Rehab Services 2124A and we will give you the door code when we receive the signed forms. OR
  Fax both forms to 5615 or interoffice mail both forms to REHAB SERVICES.
• When we receive both signed forms we will call or e-mail you the access code for the door. This code is for your use only. Please do not give this code to anyone.
• If you would like a locker please come to Rehab Services 2124A and we will issue a lock and locker number. There are only a limited number of lockers available.
