Documents required for specimen submission to PHLET for a Biothreat agent rule-out include the following:

- Environmental Submission
- Receipt of Property Form
- Chain of Custody Form
- Biothreat Clinical Submission
- Shipper’s Declaration for Dangerous Goods
- The Federal Select Agent Program(selectagents.gov) has prepared “Guidance for Completing the Shipper’s Declaration for Dangerous Goods” to assist in complying with the requirements of the select agents regulations.
ASM Sentinel Procedures
The ASM has Sentinel Laboratory procedures on their website. These include procedures for Brucella, Burkholderia, Coxiella, Novel influenza viruses, Smallpox, SEB toxin, and Francisella tularensis.
