Sports Medicine Fellowship – Apply

 

All applicants must be from a residency program accredited by the Accreditation Council for Graduate Medical Education in either emergency medicine, family medicine, internal medicine, pediatrics or physical medicine and rehabilitation.  Applicants must be eligible to obtain a permanent license to practice medicine in the state of Texas.

Our program participates in the National Residency matching Program.  All Applicants must apply through Electronic Residency Application Service.

If you meet the eligibility criteria above, and successfully submit each of the items below, you will be considered for an interview with the primary care sports medicine fellowship.

    • Personal Statement
    • State License or Permit Information
    • Three reference letters (one from current Program Director)
    • USMLE or COMLEX Scores
    • Medical School Diploma
    • Medical School transcripts
